Home Care for Cardiovascular Disorders

 
Our goal is to help patients achieve and maintain their maximum level of independence and wellbeing. By providing the appropriate education and skilled services, patients are able to safely remain in their homes once home health is no longer required.

Home care is appropriate for:
  • Recent hospitalization
  • Post-cardiac surgery
  • Post pacemaker insertion
  • Diagnosis or recent onset of disease
  • Fluctuating or unstable vital signs
  • Paroxysmal nocturnal dyspnea
  • History of frequent chest pain or changes in chest pain pattern
  • Orthopnea
  • History of syncope and fainting / frequent falls
  • Recent history of myocardial infarction/HTN/CHF/CVA
  • Recent history of hemiplegia
  • Loss of function or control
  • Initiation of new medication regimen PT/INR for medication management/coumadin therapy
  • Multiple cardiac drugs (increased fall risk)
  • Peripheral edema
  • Patient requiring dietary/fluid modifications and teaching
  • Wound care (preferred providers for KCI)

Frequent trips to the Emergency Room indicate a need for a skilled nurse to educate the patient about disease process and management
