RN OASIS QA Specialist
This position is responsible
for conducting quality assurance and operational reviews, generate reports, and
identify desirable corrective and preventative actions. Maintain current
knowledge of area program regulatory and company requirements to ensure accurate
quality assurance review.
Essential Duties and
Responsibilities:

Assures current and accurate
clinical records in compliance with documentation requirements and
program specific rules for coverage and qualifying criteria

Assures
clinical records are maintained in their original state and not
improperly altered or fabricated

Able to perform OASIS
SOC/ROC/Recertification reviews of past field staff visits
Regularity/Compliance:

Ensure compliance with all Federal, State
and local regulations and standards of practice

Complies with
Medicare, Medicaid and State license requirements

Reviews
documentation for necessity of services, quality care and compliance
with physician orders and the plan of care to ensure quality care and
reimbursement

Assures accuracy of claims by providing necessary
services with appropriate physician orders that substantiate a
reasonable and necessary service need
Requirements: OASIS
Experience: One (1) year of Home Health specific clinical experience
required. Experience conducting quality assurance and continuous improvement
activities preferred. Licensure: Current RN license in the state of
practice.

Hospice Volunteer

Physical
Therapists POSITIONS AVAILABLE
We are currently looking for well qualified Physical
Therapists interested in PRN work that are committed to the
highest standards of integrity, quality and reliability.
This is an excellent way to supplement your income. Most
visits are day or early evening with only occasional weekend
visits in the patient’s home setting.
Therapists will
provide physical therapy services to client according to a
physician’s written orders. This may include, but is not
limited to the following: Assess and evaluate;
therapeutic/rehabilitative/functional status; Develop a plan
of care and treatment making sure plan of care is consistent
with assessment; Participate in the development of the
client’s total plan of treatment; Assess the home
environment especially as it relates to fall risk and
interventions and home safety; Assess for muscle strength,
mobility, gait, and range of motion; Assess for potential
for rehabilitation; Assess for pain as appropriate for
client; Direct physical therapy treatment; Select
appropriate assistive devices; Provide gait training as
required; Develop home exercise program as needed; Instruct
client and family or caregivers in the use and care of
therapeutic appliances; Report client’s response to
treatment or changes in condition to the Director of Nursing
and physician.
LVN / LPN POSITIONS AVAILABLE
The licensed practical nurse furnishes services in
accordance with agency policies in the patient’s home,
prepares clinical and progress notes, assists the physician
and registered nurse in performing specialized procedures,
prepares equipment and materials for treatments observing
aseptic technique as required; and assists the patient in
learning appropriate self-care techniques.
LVN must be currently licensed as a licensed practical nurse
or licensed vocational nurse in the State of Texas; prefer
one year nursing experience.
LVN must comply with accepted professional standards and
practice, possesse and maintain good physical stamina and
mental health; is self-directed with the ability to work
with little supervision; has good organizational skills, is
flexible and cooperative in fulfilling role obligation; and
is a licensed driver with an automobile that is insured in
accordance with state and/or Agency requirements and is in
good working order.
HHA
POSITIONS AVAILABLE
The home health aide provides services that are ordered by
the physician in the plan of care in the patient’s home. The
duties of a home health aide include the provision of
hands-on personal care, performance of simple procedures as
an extension of therapy or nursing services, assistance in
ambulation or exercises, and assistance in administering
medications that are ordinarily self-administered.
HHA must possesse and maintain good physical stamina and
mental health; has ability to read and follow written
instructions and document care given; is self-directing with
the ability to work with little direct supervision;
demonstrates empathy for the needs of the ill, injured,
frail and the impaired, demonstrates flexibility and
cooperation in fulfilling role obligation; demonstrates
tact, patience, and good personal hygiene; and is a licensed
driver with automobile that is insured in accordance with
Agency requirements and is in good working order.
Attendants
POSITIONS AVAILABLE
Must be at least 18 and able to pass a criminal background
check.
Attendants may provide personal assistance services,
including bathing, dressing, bowel and bladder management,
transferring from bed to wheelchair, meal preparation, light
housekeeping, and other tasks as requested.
Job involves lifting and bending.
Limited amount of travel.
KNOWLEDGE, SKILLS, ABILITIES:
Assistant must be reliable, punctual, neat and organized,
willing to perform tasks as requested, willing to learn job
requirements, able to follow instructions. Looking for
someone who wants job on a long-term basis.
Training will be provided to the assistant by the
participant and/or by his family members. No prior
experience is required; must be willing to learn.
OTHER REQUIREMENTS/CONSIDERATIONS:
If assistant decides to discontinue employment, he/she must
be willing to continue working until a replacement is found,
which could be 6-8 weeks, and be willing to train
replacement.
Prefer non-smoker, no pets or children brought into
patient's home
and no personal visitors while on the job.
